Bro, I agree. Then $25 when they get to NASDAQ and then $250 after news leaked on cash back or stock split. Trust me. All this is possible. Recently the Japs really bought a large company in a pharm sector paying billions. If they want BIG it is just like plucking some hairs from their legs.

There has to be some form of clinical evidence that DES like Biomatrix / Endeavor / Xience is safer for thrombosis for doctors to believe in the first place. With this clinical evidence, it will be a good enough marketing platform for the MNC to make their marketing sales pitch. No need to 100% late-thrombosi free. No stent maker can guarantee 100% thrombosis free DES and no DES can do it. Late-thrombosis problem is a combination of many factors including patient profile, past medical history and genes although poor quality durable polymer has long been suspected to be the main culprit. Endeavor seems to be safe in their earlier trials but in Endeavor IV trial against Taxus, Endeavor's late-thrombosis cases were actually higher than Taxus although previous Endeavor trials yielded zero late-thrombosis cases. So Endeavor is not so safe after all. Endeavor's DES efficacy is only "non-inferior" to Taxus but the fact is a lot of Endeavor's clinical end-point measurements are actually slightly higher when compared against Taxus. Endeavor even failed in secondary end-point against Taxus with regards to late-loss. Endeavor's binary restenosis rate (>50% vessel reclogged) is also quite bad. Luckily a lot of such patients suffering with binary restentosis need not come back to the cath lab to do a revascularisation and Medtronic quickly capitalise this observation to play down / cover-up Endeavor's high late-loss and binary restenosis problems. To me, if using Endeavor will have a much higher chance of over 50% vessel reclogged after stenting as compared to using Biomatrix / Cypher / Xience, it doesn't sound very platable to me. People go for stenting because their vessel is over 70% clogged. After stenting and the vessel still has a high chance of over 50% of vessel reclog? What kind of DES is this? A bit scary also. Anytime if reclog a bit more i may have to go back and do another stenting ops. Despite of this clear and present reclogging risk, Medtronic still can capitalise on just this "one and only but questionable" low late-thrombosis selling point and make a fortune out from it. Xience always has late-thrombosis cases for all their previous clinical trials but at a lower rate compared to Taxus. Just this lower late-thrombosis rate is already a good marketing point. Plus, Xience slam dunked Taxus for DES efficacy. That is why Xience is more highly regarded clinically and touted to be the next DES market leader. JNJ will study the LEADERS 12 month data, OCT scan, sub-group analysis very carefully. In terms of DES efficacy, Biomatrix is confirm in the top league together with Cypher and Xience. I personally feel Biomatrix is slightly above Xience and Cypher in terms of DES efficacy. For late-thrombosis, Biomatrix is confirm in the top league together with Endeavor and Xience. However, the fact that Biomatrix uses PLA biodegradable polymer VS durable polymer means Biomatrix has an advantage over these 2 other DESs. No one can doubt the long term safety profile of a BMS because they have been in use for over 20 years without any late-thrombosis problems. Durable polymer has always been a prime suspect (though not the only cause) of late-thrombosis. Why still want to leave a durable polymer inside a vessel after its drug eluting function is over after a few months? The durable polymer is of absolutely no use after its drug eluting function is over within a few months after stenting. Polymers are more "fragile" by nature and always has potential problems like molecular degradation, crack, inflamation, vessel sensitivity to polymers, etc after many years. The more questionable elements in the vessel, the more potential problem may happen years later. Always better cut down unnecessary risks and make thiings simple and safe. Can definitely rely on BMS safety profile as BMS has been proven to be reliable for 20 years with regards to late-thrombosis issues. This is the main selling point of Biomatrix against newer DESs like Endeavor and Xience. Best in efficacy, reverts back into a BMS which has 20 years safety track record. No second guessing of what will happen to the durable polymer that is still remaining inside the vessel. |

Once bitten, twice shy? JNJ has paid dearly for their Connor mistake. Will they be more cautious in the future? Solving the late stent thrombosis issue is the key. Xience has shown to be effective. Is it also safe? Biomatrix has shown to be effective. Will it also be safer than them all? These are the billion dollar questions, and only time will tell.

Also, Costar didn't prove "superiority" against any other DESs. JNJ bought Conor just because of the novel reservoir hole DES technology. Likewise, the PLA biodegradable polymer and Totally Polymerless has very strong clinical evidence and potential to handle the late-thrombosis problems. Late-thrombosis problem is so serious that DES usage in USA dropped from a high of 90% vs BMS usage to a low of around 62% vs BMS usage. Thats almost a 33% sales drop just because of late-thrombosis!!! BIG's DES biodegradable and polymerless DES technology is a great marketing platform to capture market share. The best part is Biomatrix's DES efficacy is much better than Endeavor which is perceived to be a safe stent but poor in DES efficacy. BIG's DES technology has filled the gap of BOTH solid DES efficacy and longer term safety with regards to late thrombosis. Best of both worlds. Just this aspect will make Biosensor's Biomatrix DES and Biomatrix Freedom (totally polymerless) DES widely popular DES if they marketed well by a powerful MNC. |
